Registration

To participate in the program, every participant needs to register online with ezWaiver for tickets, rentals and/or lessons by the dute date prior to each trip.  Then, submit payment if paying with cash/check and permission/agreement forms to Carrie Smith at the Custer County School office.  If paying with a credit card, you will need to call Carrie Smith with credit card information to submit to Monarch.  

Follow the instructions below to register for each trip.

2. Submit to Carrie Smith at Custer County School:

If you only need to ride the bus and do not need tickets, rentals or lessons, you must still fill out and submit the Permission Form and Custer County Agreement.

 –  ezWaiver General Instructions  –

1.  Read over the ezWaiver Do’s and Don’ts section beneath the link ↓.
2.  Click on ezWaiver.
3.  Select the Get Started button or Go To Login button to log back in to an existing account.
4.  Choose the option that best describes who you are filling out a waiver for.
5.  Identify yourself first.
6.  Confirm your personal information.
7.  Create your signature and initials.
8.  Sign and save your waiver.
9.  If adding a minor, click Add A Minor.

  • Confirm your minor’s information
  • Sign and save your minor waiver
  • Repeat step 9 for additional minors

10.  Make sure to accept and apply your signatures.
11.  You will receive a conformation email once your waiver has been completed.

 –  ezWaiver Do’s and Don’ts  –

All Participants:

  • Every participant must complete and sign this liability release in advance.
  • Please have all information in ezWaiver a minimum of 7 days prior to arrival so we can ensure all the appropriate reservations are made for tickets, rentals and lessons.
  • All adults must sign for themselves.
  • Once your waiver is generated, you must still apply your signature to the document and save.
  • You will know that your waiver is complete when you receive a confirmation email.

Minors:

  • An adult must sign for participants younger than 18 years of age.
  • If you have minors, you can add them after you enter your information and choose whether or not you are participating.

Account Information:

  • Your password can be reset if you forget it.
  • The group leader has the ability to check waivers if you need to confirm information.
  • Changes cannot be made directly to your release form once you have signed it so to make changes please call a Monarch Group Sales staff member at (719) 530-5102.
  • Changes/additions made to reservations within 48 hours prior to your arrival will cost $5.
